Federation of State Massage Therapy Boards Massage & Bodywork Licensing Examination (MBLEx) 866-962-3926 Please complete an MBLEx Result Transfer Form to have your scores sent to Louisiana. This form may be found on the website (www.fsmtb.org).
If you need to reapply to take the MBLEx, click here to print the form to submit with the Louisiana Application for License.
