Establishment Applications
The LBMT web development team is currently working to complete all functions of the new website. Currently ONLINE functions for Establishment Licenses and Establishment Registration (solo practitioner) applications are not available. Please review the below to determine which application is needed. Download the application, complete and return to the office. Requirements: Therapist who work from home, offer mobile massage services, or those who work in a physician’s office, physical therapy facility, chiropractic office or higher education sports facility are the only licensees that are NOT required to be listed on an establishment license, have an establishment license or registered with an establishment license as a solo practitioner. If you do not fall under any of the above listed, you are required by law to have an establishment license, be listed as working at a licensed establishment, or be registered as a solo practitioner with an establishment registration.

January 2026: In accordance with with ACT 453 (SB332) all NEW Establishment Licenses applicants will be required to submit a background check with their application.
- Completed a criminal history background check as set forth in R.S. 37:3555(A) (14)
- Criminal history background must be for the registered owner(s) of the establishment listed on the application and business registration paperwork.
- Documentation showing applicant has the right to possess the premises where establishment will be located. (lease, property ownership, letter of intent from landlord)
- If change of ownership – documentation showing sale or transfer – business closure from must be received from previous owner
- Completed all requirement of the application process
- All applicable fees

§2701. Inspections
- The board will inspect all massage establishments, including licensed and/or unlicensed massage establishments.
- Such inspections may include, but need not be limited to, confirmation that the site is being utilized for massage therapy and a determination of whether the establishment is in compliance with the laws and rules governing the establishment’s operation, facilities, personnel, safety, and sanitary requirements.
- Failure to cooperate with such inspections will result in violation fines and may lead to disciplinary action.
- Fines and violation(s) processes are located in the Fines & Penalty Schedule
The LABMT adopted a Fines and Penalty Schedule in 2013 to be uniform in the administration of fines and penalties and to address violations noted on any inspection report, office audit or otherwise brought to the attention of the Board. Minor violations found from required office audits such as required filings, registrations or update notifications result in fines starting at $100.00 per violation. Examples include but are not limited to, change of address, working location etc.
Inspection violations based on the violation of any statute, rule or regulation start at $300.00 not to exceed $750.00. Serious inspection violations are $1000.00 per violation. To review the full Fines and Penalty schedule click here
Serious inspection violations, including but not limited to:
- a massage establishment being used as a principal or temporary domicile, shelter, or harbor, or as sleeping or napping quarters for any person unless the establishment is zoned for residential use under a local ordinance
- Aiding, assisting, procuring, or advising any unlicensed person to practice massage therapy, contrary to this rule or to a rule of the department or the board
Repeat, subsequent violations or failure to show corrective action, will result in double fines and or disciplinary action. To review the fines and penalty schedule please see the law tab on the website.
If a licensee desires to contest any violation issued by the Board, Notice of Intent to Contest the violation must be provided in writing to the Board office within 15 days the date of the Notice of Violation. A Notice of Intent to Contest form shall be found on the LBMT website or is available from the Board office. Upon receipt of such notice to contest, the contesting party will be advised as to the right to resolve the matter with the office or request a formal hearing with the Board. If a hearing is requested, the contesting party will be provided with a Notice of Formal Disciplinary Hearing, which will include the date, time and location of the hearing, right to representation, and options as to how the hearing will be conducted.
