The LBMT web development team is currently working to complete all functions of the new website. Currently ONLINE functions for License Registrations are not available. Once a professional license application is approved registration is required
- Approved applicants must register their license within 45 days from the date in which the application was approved. If past 45 days, the application process will need to be completed again.
- The registration fee is determined by the month in which the registration is submitted

Once an Establishment License application is approved registration is required
- Approved applicants must register their license within 45 days from the date in which the application was approved. If past 45 days, the application process will need to be completed again.
- The registration fee is determined by the month in which the registration is submitted
$100.00 April to August / $80.00 September to December / $60.00 January to March - Download the ESTABLISHMENT LICENSE registration here
Once a professional Lapsed license application is approved registration is required
- Approved applicants must register their license within 45 days from the date in which the application was approved. If past 45 days, the application process will need to be completed again.
- The registration fee is $575.00 with 24 CEU’s

- Provisional Licenses are valid for a three-month period only.
- The supervising/sponsoring of a Provisional Massage Therapist shall endeavor to aid in the professional development of the licensee to ensure that the Licensee complies with the ethical and legal standards in providing massage therapy services to the general public
- The three month provisional license also allows ample time for the licensee to complete and pass the national exam required for professional licensur Provisional Licensees must complete the Provisional License application and meet all requirements as stated on the application.
- No Licensee shall work before the issue date on the Provisional License or after the expiration dat If the national exam is not passed on or before the expiration date of the provisional license, the licensee with be considered a new applicant and will be required to meet all new applicant requirements upon passing of the national exam, including a new application
