The LBMT web development team is currently working to complete all functions of the new website. Currently ONLINE functions for License Registrations are not available. To register your professional or establishment license download the correct form below, complete and return to the office. For questions email info@labmt.org or call 225-756-3488
Once a professional license application is approved registration is required
- Approved applicants must register their license within 45 days from the date in which the application was approved. If past 45 days, the application process will need to be completed again.
- Registrations can be done online through your license management portal
- The registration fee is determined by the month in which the registration is submitted
