CEU Provider Course Renewal and New Course Submissions: Reminder: It is the providers responsibility to ensure that each certificate has listed: Provider's Name, Provider Number, Instructor Name, LCEU Course Number, Title of the Program, Completion Date, CEU Hours, Licensee's Name & License Number. Certificates that do not have the above will be returned to the licensee. Chapter 37 - Continuing Education 2. The provider must furnish each participant with a certificate or letter of attendance verifying that the program has been completed. The certificate shall contain the provider's name and number, the title of the program and instructor, the program number assigned by the board, the date, the number of CEU hours, and the licensee's name and license number.
