To complete the instructor application online, create an LABMT profile and complete the application.  You will be notified via email after review of your application. 

The application can also be printed and mailed to the LABMT office.  This Application must be completed in full by the applicant. Incomplete applications will be returned to the applicant for completion. This will result in a delay of receiving your certificate and a delay in being eligible to instruct.  Only money orders will be accepted.  NO cash or checks are accepted.

You must include all applicable paperwork that will help us to determine your qualifications for certification. This includes:

  • An official transcript from the institution in which qualifying education was received,
  • A copy of any licensure currently held by the applicant,
  • Copies of all Continuing Education relevant to courses desiring to be certified, and
  • A current copy of the applicant's resume.

List all courses that you would like to be approved to teach. The Board will only consider approving those courses listed. If the course is not approved it will not be listed on the certificate.
